DENTAL INSURANCE

DENTAL INSURANCE PLAN BENEFITS

Below is our CBA Schedule of Dental Benefits.  

Our Dental Insurance is completely FREE to ALL full-time (1.0 FTE) employees for Single, 2-Adults, Parent/Child(ren), & Family Plans.  

Eligible dependent children can remain on your dental insurance plan as long as they are under 19 OR if they are under 25 & enrolled full-time in college.  

Our Dental Insurance plan year runs from July 1 to June 30.  

Every July 1st is our Supervisory Union's Annual Open Enrollment Date for Dental Insurance.    

DENTAL INSURANCE PLAN ENROLLMENT/CHANGES

Any Employee who has had a qualifying event & would like to either Enroll or Change their current Dental Insurance Coverage, must complete this CBA Dental Insurance Enrollment/Change Form & email it to Candy Granger from your FWSU email address.  Forms will be processed within 10 Calendar Days & only emailed forms will be accepted.

LIFE INSURANCE

LIFE INSURANCE PLAN BENEFITS

All Employees who work at least 20 Hours/Week are automatically enrolled in our FREE Employer Paid Term Life Insurance.  

LIFE INSURANCE - BENEFICIARY DECLARATION FORM

Any employee who will be enrolled in our life insurance or who wants to change their current beneficiaries, must complete the Life Insurance - Beneficiary Declaration Form below & email it to Candy Granger from your FWSU email address.  Forms will be processed within 10 calendar days & only emailed forms will be accepted.

RETIREMENT - VMERS & VSTRS

VMERS: Vermont Municipal Employees' Retirement System

VMERS is the mandatory Vermont Municipal Employees' Retirement System for any employee who does not have a Vermont Teaching License & regularly works:

  • 30 or more hours per week throughout the School Year OR 

  • 24 or more hours per week throughout the Calendar Year.  

These employees are Paraprofessionals, Office Staff, Tech Staff, Custodial/Maintenance Staff, Business Managers, OT, BCBA, etc.  The state of Vermont sets both the Employee & Employer Contribution Rates, which usually both increase slightly every July 1st.

Below are each of our 4 entities' Notification of Employment/Mandatory Enrollment forms.

VSTRS - Vermont State Teachers' Retirement System

VSTRS is the mandatory Vermont State Teachers' Retirement System for any employee working as a Teacher or Administrator with a Vermont Teaching License.

These employees are Teachers, Nurses, SLPs, Principals, Superintendents, etc.  The state of Vermont sets the Employee Contribution Rates based on your salary & these rates usually increase slightly every July 1st.

VMERS & VSTRS - Designation of Beneficiary Form

Any Employee Enrolled in VMERS or VSTRS who wants to change their current beneficiaries at VMERS or VSTRS, must complete this VMERS & VSTRS - Designation of Beneficiary Form.

You will then need to get this form notarized before you mail it to the Vermont Retirement System at the address at the top of this form.
